HomeCare Elite Designation Reflects Quality Care, Patient Experience

Colchester, VT–October 16, 2018 – The University of Vermont Health Network – Home Health & Hospice (formerly VNA of Chittenden and Grand Isle Counties) has been named a 2018 HomeCare Elite agency, a designation awarded to the top-performing home health agencies in the United States. This year, 8,898 home health agencies were considered, resulting in Elite recognition for 2,223.

In partnership with Decision Health, ABILITY Network’s HomeCare Elite ranking program performs a comprehensive analysis of all home health care agencies in the country evaluating their overall performance to identify the top 25 percent of all Medicare-certified home health agencies in the United States.

HomeCare Elite agencies are determined by an analysis across an array of performance measures in quality outcomes, best practices implementation, patient experience, quality improvement and consistency and financial health. To be considered, an agency must be Medicare-certified and have data for at least three outcome measures in Home Health Compare – a program created by the Centers for Medicare & Medicaid Services (CMS) to provide the public with information about the quality of care provided by Medicare-certified home health agencies.

UVM Health Network – Home Health & Hospice, formerly the VNA of Chittenden and Grand Isle Counties, has recently changed its name to reflect its affiliation with the UVM Health Network.
