How to Refer a Patient

The UVM Health Network – Home Health & Hospice provides services for residents of Chittenden and Grand Isle Counties in Vermont. Our McClure Miller Respite House, Vermont’s only Medicare-certified inpatient hospice residence, serves individuals and their loved ones from all over Vermont and beyond.

Please download and complete the form for referrals to home health care and return by fax to the Intake Office at 802-860-4464. Please note: Our hospice referral form is currently being updated.

For all other programs and services, call our Intake and Referral team or refer to the contact information on the program pages on this website.

By Telephone

Our Intake and Referral team is available weekdays 8 am to 5 pm. Call 802-860-4400 or toll free at 1-800-427-1908.

For nights, weekends and holidays, a UVM Health Network – Home Health & Hospice Intake nurse is available from 7 am to 3 pm.

For referrals after-hours when you do not need to speak to a UVM Health Network – Home Health & Hospice nurse, we offer an after-hours confidential voicemail system at 802-860-4484.

Please leave your referral request and an Intake nurse will call you (or someone whom you designate) on the next business day to confirm receipt of the referral. Please speak slowly and include the following information:

• Your Name
• Telephone number where we can call you back
• Patient name you are referring (please spell the name) and their date of birth
• Department or floor if you are calling from a hospital or physician office
