Making a gift to honor your caregiver or giving in memory of a loved one is a great way to give back to our community.

Honor a Caregiver

There’s no better way to acknowledge a wonderful caregiver than through our Honor a Caregiver program, which allows you to support Home Health & Hospice in the name of a special nurse, social worker, therapist, or multitude of staff who stood by you throughout your home health or hospice journey.

Our Honor a Caregiver program allows you to pay tribute to a Home Health & Hospice team member who offered especially meaningful comfort and care throughout your or a loved one’s experience. After you give a gift to honor a designated caregiver, they will receive a notification and their incredible care will be recognized by Home Health & Hospice leadership and team members. Additionally, they will receive a “Healthcare Hero” pin to wear proudly on their ID badge – letting our whole community know about your caregiver’s heroic efforts.

How to Honor a Caregiver

  1. Navigate to the Donate page: uvmhomehealth.org/give
  2. Complete the General Information, Donation Information and Area You Wish To Support sections.
  3. Under the Optional Attribution section, click the green plus button next to “I wish to thank a caregiver.”
  4. Add the name and a message for the caregiver.
  5. Click the orange Donate button at the bottom of the form.
  6. You will be directed to our PayPal account to complete the donation.

Memorial Giving

Memorial donations are donations given in memory of someone who has passed away. Memorial gifts are a beautiful way to honor someone’s life because the gifts provide compassionate care to other community members who might not be able to afford the care that they receive.

Obituary Donations

Including Home Health & Hospice in your loved one’s obituary is a beautiful way to make an impact in the community. The following text has all of the necessary information and can be included at the end of an obituary:

In lieu of flowers, memorial donations can be made to UVM Health Network – Home Health & Hospice (or a specific program, such as McClure Miller Respite House), 1110 Prim Road, Colchester, VT  05446 or give online at www.uvmhomehealth.org/give.

We will thank each donor who gives in memory of your loved one and provide you with names and addresses if you wish to personally thank them.

Planned Gifts

You can make a future gift to our organization in memory of a loved one. Planned gifts such as including us in your will or estate plan or including Home Health & Hospice as the beneficiary of your IRA allow you to arrange for future gifts to UVM Health Network — Home Health & Hospice.

To donate through our Honor a Caregiver program or to give in memory of a loved one, you may either make your gift online or explore one of our additional ways to give.

To mail a gift, please send your cash or check to the following address and include a note with the caregiver’s full name and/or who the gift is in memory of.
