With your support, the UVM Health Network – Home Health & Hospice has been able to provide home health services to 5,500 of the most vulnerable people in our community each year.

With reductions in Medicaid and Medicare revenue, your support is needed now more than ever to ensure the UVM Health Network – Home Health & Hospice can be there every step of the way to meet the complex health needs of the entire family.

Our Family & Children Services Program is there for new mothers like Danielle who experienced a traumatic pregnancy when the cord was wrapped around her daughter’s neck. The UVM Health Network – Home Health & Hospice started home visits just two weeks after the delivery to ensure that her daughter didn’t experience development delays due to the trauma.

Adult Home Care helps people heal at home from surgery, such as Tom, a 35-year-old heart transplant recipient who started receiving home health care before his transplant and continued to receive visits once a week post-surgery. With regular visits, education and guidance from UVM Health Network – Home Health & Hospice nurses, Tom has been able to stay at home, has the confidence to manage his own condition and is now a spokesperson for people with cardiac disease.

During the most difficult times, our End-of-Life Care programs support families like Roger and his wife, Susan. Susan suffered from an aggressive form of brain cancer and received home hospice and inpatient hospice care at the UVM Health Network – Home Health & Hospice Respite House during the final days of her life.

Last year’s charitable care totaled over $1.8 million. Every donation, no matter the size, ensures the UVM Health Network – Home Health & Hospice can continue to serve all who turn to us for nursing care, rehabilitation services, long-term care and end-of-life support, enabling people to live life to the fullest.
