Our nurse-led teams provide skilled home health care services and education to help you or your loved one who is learning to cope with the management of an illness or disease.

Our specially-trained interdisciplinary team of care providers will coordinate with you and your physician to establish goals and tailor a comprehensive plan of care to meet your needs. Through coaching, education and advanced technology like telemonitoring, we’ll help you and your loved ones learn self-management skills to help you feel more confident with living with your disease.

What types of illnesses or diseases can be helped by these services?
We specialize in the recovery care and treatment of many chronic illnesses and diseases, such as:
- Cancer

- Infections or Pneumonia

- Gastrointestinal trouble

- Neurologic disease such as Parkinson’s, MS or ALS

Who provides the care?
Our interdisciplinary team of home health professionals includes Registered Nurses, Licensed Nurses Assistants, Licensed Practical Nurses, Medical Social Workers, Intake and Hospital Discharge Nurses and Personal Care Attendants. All care is overseen by your physician.
When is home health care available?
We are available seven days a week, 24 hours a day, with day and evening visits scheduled and on-call services at night.
How do I access home health services?
A patient or family member can request home health care. However, Medicare, Medicaid and private insurances require a physician’s order for certified home health. If your physician agrees you would benefit from home health, he/she will make a referral to a certified home health agency and will authorize a treatment plan, commonly called a plan of care. How do I pay for home health services?
Home health services are covered by most insurance, including Medicare, Medicaid, and private insurers (for example: Blue Cross & Blue Shield, MVP, or Cigna) when your doctor certifies that:
- You will benefit from medical care at home, and
- You are homebound, and
- You need care from a registered nurse or a licensed physical, speech or occupational therapist
